These diseases are frequent comorbidities, resulting in additional symptomatic burdens and higher socioeconomic costs in patients with both conditions.1 Both conditions are characterized by a strikingly similar underlying pathology; i.e., inflammation of the respiratory mucosa associated with the same allergic and pro-inflammatory mediators and cells.1,2 This slide kit will discuss the epidemiologic and pathologic links between these diseases and present data from clinical studies aimed at treating both conditions concomitantly.

Tierhaare sind ein weiteres wichtiges Hausallergen, das an beiden Erkrankungen beteiligt ist.8,26 Insekten wie Schaben sind häufige Auslöser sowohl von allergischer Rhinitis wie Auch Asthma.16,27 Tabakrauch, eine Hauptquelle der Verschmutzung von Innenräumen kann Asthma oder allergische Rhinitis auslösen, gleichgültig ob der Patient selbst raucht oder Rauch von anderen einatmet.16,27 Gemeinsame Freiluftallergene sind Pollen und Schimmelpilze.16,27

5 Asthma und AR* basieren auf einer Entzündungsreaktion

Slide 11 Asthma is a chronic inflammatory disorder in which exposure to various stimuli results in airway obstruction and airflow limitation.16 Allergic rhinitis is an immunoglobulin E (IgE)–mediated inflammatory disorder in which exposure of the nasal membranes to allergens leads to nasal symptoms.7 Although allergic rhinitis and asthma traditionally have been diagnosed and managed as distinct upper- and lower-airway diseases, respectively, recent insights into their underlying pathophysiology have identified a continuum of inflammation within the upper and lower airways.17 Several new terms have been proposed to describe the ubiquitous airway inflammation observed in patients with concurrent allergic rhinitis and asthma. These include “one airway disease,”17 “allergic rhinobronchitis,”18 “united airways,”19 and “one linked airway disease,”20 among others. **IgE = Immunglobulin E *AR = Allergische Rhinitis

Die Grundlage beider Reaktionen ist die unmittelbare Hypersensitivität, hervorgerufen von einer Sturzflut von Ereignissen, die mit der Sensibilisierung und der Ausschüttung von allergenspezifischem IgE beginnt, wenn ein Allergen mit membran-gebundenem IgE an der Oberfläche von Mastzellen interagiert. Diese Interaktion löst eine Degranulation der Mastzelle aus, d.h. die Ausschüttung präformierter Mediatoren (Histamine und Tryptase), Cysteinyl-Leukotriene, Prostaglandine und des platelet-aktivierenden Faktors (PAF), die direkt für die Frühphase der Reaktion verantwortlich sind. Die Spätphase der Reaktion wird primär duch die Aktivierung von Mastzellen oder T-Zellen in Gang gesetzt, die die Ausschüttung von Cytokin bewirkt. Cytokine beeinflussen eine weite Palette von mit chronischer Entzündung zusammenhängenden Ereignissen zusammenhängen, darunter Einsatz von Eosinophilen sowie die daraus folgende Ausschüttung von Cysteinyl-Leukotrienen und anderen neu gebildeten Entzündungsmediatoren.21, 28, 29 Während sich die präformierten und neu gebildeten Entzündungsmediatoren in den oberen und unteren Atemwege ähneln, kann ihre Wirkung in den verschiedenen Zielorganen eine unterschiedliche sein.18 Eosinophile T- Zellen Entzündungs- mediatoren Zytokine Spätreaktion

Einwanderung von Eosinophilen Allergische Rhinitis Asthma Dia 14 Nasen- und Bronchialgewebe besitzen ähnliche histologische Eigenschaften, darunter ein pseudo-geschichtetes Epithel mit säulenförmigen Wimpernzellen, die bei normalen Personen auf einer Basalmembran ruhen.7 Es erstaunt daher nicht, dass der Entzündungsprozess in beiden Geweben auffällend ähnlich verläuft: sie sind der durch die Ausschüttung von Mediatoren wie Cysteinyl-Leukotrienen und Cytokinen während der allergischen oder asthmatischen Reaktion bewirkten Eosinophil-Infiltration ausgesetzt. Eosinophile können die bei beiden Krankheiten beobachtete Gewebsschädigung fördern und die allergische und entzündliche Reaktion aufrechterhalten und weiter verstärken.29 Da es in der Struktur der Schleimhäute der oberen und unteren Atemwege Unterschiede gibt (z.B. die größere Durchsetzung des Nasengewebes mit Gefäßen, das Vorhandensein glatter Muskulatur in den Bronchien und der größere Epithelabbau in den Lungen), löst die Entzüngung in diesen Zeilgeweben bei allergischer Rhinitis und bei Asthma eine unterschiedliche Palette von Symptomen aus. Dazu gehören Bronchokonstriktion in den Lungen und Gefäßerweiterung, die zu Atembehinderungen in der Nase führt.29 Nasenschleimhaut Bronchialschleimhaut Eos = Eosinophile; Neut = Neutrophile; MC = Mastzellen; Ly = Lymphozyten; MP = Makrophagen

Etwa eine Stunde nach der Allergenexposition erreichen die Symptome bei Patienten mit allergischer Rhinitis einen Gipfel, während es bei Asthmapatienten zu einem starken Abfall der Lungenfunktion (gemessen durch forced expiratory volume per second [FEV1]) kommt. Sixty adult patients with house dust–induced allergic rhinitis were assigned to nasal provocation with LTD4 (n=14), histamine (n=14), antigen (n=18), or a nonactive control solution (n=14). Nasal airway resistance (NAR) was expressed as the ratio of peak NAR with the active agent to peak with the control solution. The endpoint of concentration (threshold) was the minimum concentration of the active agent required to produce an increase of at least 20% in sneezing, nasal secretion, or NAR compared with nonactive control provocation.48 LTD4 had longer-lasting effects on NAR than histamine. After histamine challenge (n=11), NAR increased once at 1 hour (p<0.05 vs. baseline) and returned to baseline between one and three hours on average. After LTD4 challenge (n=7), in contrast, NAR increased significantly at 30 minutes (p<0.05) and did not return to baseline for 11 hours. Moreover, LTD4 was a stronger mediator of nasal response than histamine, as reflected by a threshold concentration approximately 5000-fold lower than that of histamine. Cysteinyl leukotriene concentrations in nasal secretions were measured at pre-challenge and after challenge with a lactose diluent to establish baseline levels, and 10 minutes after each challenge with 10, 100, 1000, and 5000 grains of ragweed pollen.20 In the subjects with allergisch rhinitis, mean cysteinyl leukotriene concentrations in nasal secretions showed significant dose-responsive increases; i.e., each dose of pollen resulted in a significant increase in leukotriene production compared with levels after the preceding dose and baseline levels (10 grains, p=0.03; 100 grains, p=0.004; 1000 grains, p=0.02; and 5000 grains, p=0.01). Cysteinyl leukotriene concentrations were correlated significantly (p<0.001) with clinical response (sneezing).20 In non-allergisch control subjects, cysteinyl leukotriene levels did not increase after any dose of ragweed pollen.20 These findings demonstrate that cysteinyl leukotrienes are produced in individuals with allergisch rhinitis during acute allergisch reactions, and that their production is correlated with clinical response (symptoms).20 The absence of these patterns in non-allergisch control subjects provides evidence of the pathophysiologisch relevance of cysteinyl leukotrienes in allergisch rhinitis. p=0,03* (n=17) Ausgangs-wert Laktose als “Provokations-lösung” 10 Pollen 100 Pollen 1000 Pollen 5000 Pollen Provokation mit Beifußpollen Creticos PS et al. LTC4 in nasopharyngeal secretions was measured at baseline (approximately two weeks before peak pollen counts), during peak ragweed exposure, and two weeks after the end of the ragweed season. All study participants rated rhinorrhea, nasal congestion, and sneezing on a four-point scale (0=none; 1=mild; 2=moderate; and 3=severe) in daily diaries.29 In the children with allergisch rhinitis, mean LTC4 concentrations during the peak ragweed season increased significantly to 5.52±0.07 ng/ml compared to the pre-season baseline of 1.87±0.43 ng/ml (p<0.001). Individual concentrations of LTC4 in these children increased up to sixfold during the peak season versus preseason baseline. 36 Schlussfolgerungen zum Ein-Atemweg-Konzept
Allergische Rhinitis und Asthma haben viel gemeinsam: Die allergische Rhinitis erhöht die Morbidität und den therapeutischen Aufwand bei Asthmapatienten. Bei Vorliegen von Asthma und AR empfiehlt ARIA einen kombinierten Therapieansatz der oberen und unteren Atemwege. Leukotriene sind wesentlliche Entzündungsmediatoren bei Asthma und bei allergischer Rhinitis. Bei Asthmapatienten mit allergischer Rhinitis verbesserte der Leukotrien-Antagonist Montelukast 10 mg sowohl die Asthma- als auch die Symptome der allergischen Rhinitis signifikant. Dia 49 Allergic rhinitis and asthma are inflammatory disorders that have been linked epidemiologically, pathophysiologically, and clinically as “one airway disease.”7,16,21 The clinical importance of this association has been established in outcomes research showing that the presence of allergic rhinitis increases morbidity, drug costs, and use of health-care resources in patients with asthma.14,15 Therefore, current ARIA guidelines recommend a combined strategy for the management of coexistent allergic rhinitis and asthma.7 In light of the importance of cysteinyl leukotrienes as mediators of both allergic rhinitis and asthma, recent investigations have focused on the effects of cysteinyl leukotriene blockade in the treatment of patients with both conditions.1 In clinical studies, the cysteinyl leukotriene receptor antagonist montelukast was shown to improve lung function, symptoms, and quality of life in asthma patients with concomitant seasonal allergic rhinitis.1,66 Therefore, in asthma patients with seasonal allergic rhinitis, montelukast represents a therapeutic option that provides overall clinical benefits.1
