Nach 50 Jahren adjuvanter Chemotherapie des Mammakarzinoms gibt es keinen etablierten prädiktiven Faktor für den selektiven Einsatz einer Chemotherapie !

Indikation zur primären Chemotherapie Oxford AGO LOE / GR Absolute Indikationen: primär inoperables Karzinom 1c A ++ inflammatorisches Karzinom 1b B ++ Optionale Indikationen alternativ zur adjuvanten Therapie: Mastektomie aus onkologischer/ kosmetischer Sicht notwendig, aber Wunsch nach Brusterhaltung 2b C ++ Rezeptor-negatives Karzinom 2b C ++ Die Indikationen für die primäre systemische Therapie (PST) sind das primär inoperable Mammakarzinom, das inflammatorische Mammakarzinom aber auch zunehmend das operable Mammakarzinom mit einem Durchmesser über 2 cm. Wenn die Mastektomie bei der Erstdiagnose wahrscheinlich erscheint, ist die PST eine valide Option, wenn die Patientin den Wunsch nach primärer Brusterhaltung äussert. Der hormonrezeptornegative Status im Primärtumor lässt eine deutliche Verkleinerung des Tumors bis hin zum kompletten Verschwinden in bis zu 30-40 % der Fälle voraussagen und ist per se eine gute Indikation für die PST unabhängig vom Alter der Patientin.
