Die Ludwigshafenerin ist nun die erste, deren Fall vorm obersten deutschen Zivilgericht, dem BGH, geprüft wird. Ihr geht es hauptsächlich um die Verantwortung des TÜV: "Ich habe Vertrauen gehabt eigentlich in den TÜV. Ich fühle mich betrogen, dass so etwas überhaupt passieren kann. Ich gehe davon aus, dass so was in Ordnung ist, dass es untersucht ist - vom TÜV staatlich geprüft. PIP-Chef Jean-Claude Mas wurde 2013 in Marseille zu vier Jahren Haft, einer Geldstrafe und einem Berufsverbot verurteilt. Aber die deutschen Gerichte lehnten bislang eine Verantwortung des TÜV ab. Sicherheitsprofil der Implantate Denn wir implantieren Material mit der Vorstellung, dass es weit über 40 – 50 Jahre funktionieren soll. In einem biologischen System mit einem enormen Potential Fremdkörpern zu begegnen und wer kann schon vorhersagen was der Organismus in diesen Jahren mit den Implantaten veranstaltet. 2006 genehmigte die FDA 2 Implantatherstellern den weiteren Einsatz ihrer Produkte. Voraussetzung dafür war die Teilnahme an dem Studienprotokoll zur Zulassung der FDA. 1) Core Post-Approval Studies (Core Studies) – To assess long-term clinical performance of breast implants in women that enrolled in studies to support premarket approval applications. These studies were designed to follow women for 10 years after initial implantation. (2) Large Post-Approval Studies (Large Studies) – To assess long-term outcomes and identify rare adverse events by enrolling more than 40,000 silicone gel-filled breast implant patients and following them for 10-years. (3) Device Failure Studies (Failure Studies) – To further characterize the modes and causes of failure of explanted devices over a 10-year period. (4) Focus Group Studies – To improve the format and content of the patient labeling. (5) Annual Physician Informed Decision Survey (Informed Decision Study) – To monitor the process of how patient labeling is distributed to women considering silicone gel-filled breast implants. Bleeding 1. Silikongel: Gele unterscheiden sich das die flüssige Phase neben der festen Phase unterschiedlich gehalten wird („Kohäsivität“). Auch kohäsives Gel neigt zu Bleeding (niedermolekulare Bestandteile) 2. Barriereschicht: Hüllen sind schichtweise hergestellt, durch einen Tauchprozess oder durch ein Übergießen der Form. Meist Xylol und Silikonmolekülen. Mehrere Schichten werden abgedampft und dann vulkanisiert.

Diagnostik

The Effect of Breast Implants on Mammogram Outcomes. Breast cancer detection in women with implants has been questioned. We sought to evaluate the impact of breast implants on mammographic outcomes. A retrospective review of women undergoing mammography between March 1 and October 30, 2013 was performed. Demographic characteristics and mammogram results were compared between women with and without breast implants. Overall, 4.8 per cent of 1863 women identified during the study period had breast implants. Median age was 59 years (26-93). Women with implants were younger (53.9 vs 59.2 years, P < 0.0001), had lower body mass index (25.4 vs 28.9, P < 0.0001), and were more likely to have dense breast tissue (72.1% vs 56.4%, P = 0.004) than those without. There were no statistically significant differences with regards to Breast Imaging Recording and Data System 0 score (13.3% with implants vs 21.4% without), call back exam (18.9% with vs 24.1% without), time to resolution of abnormal imaging (58.6 days with vs 43.3 without), or cancer detection rate (0% with implants vs 1.0% without). Because implants did not significantly affect mammogram results, women with implants should be reassured that mammography remains useful in detecting cancer. However, future research is required to determine whether lower call back rates and longer time to resolution of imaging findings contribute to delays in diagnosis in patients with implants. Alle 2 Jahre wurden die Implamtatträgerinnen per MRT untersucht.Und es konnte gezeigt werden, dass das MRT eine sehr hohe Vorhersagekraft hat, gefolgt vom Ultraschall und er dann der Mammographie. IN sehr geübter Hand erreicht die Qualtät der US Untersuchung auch die Werte der MRT Untersuchung In dieser Review Arbeit zeigt sich, dass das MRT die höchste Sens und Spez in der Diagnose der Leckage hat. Gefolgt vom US. In den Arbeiten von Heden wurde die klinische Untersuchung zur Erfassung der Rupturrate ebenfalls untersucht und es fanden sich in den Fällen mit Ruptur vereinzelt Kapselfibrosen Becker 3, sonst keine weitern Beschwerden. 150 patients for a total of 300 implants underwent breast US and subsequently MR examination searching for signs of intra or extra-capsular rupture RESULTS: 170/300 (57 %) implant ruptures were detected at US (intra-capsular n = 110, extra-capsular n = 60). By comparing US findings with MR results, overall sensitivity, specificity, accuracy, PPV, and NPV of 79, 63, 70, 65, and 77 %, respectively, were found for breast US. In case of intra-capsular rupture, sensitivity, specificity, accuracy, PPV, and NPV of 63, 63, 63, 45, and 77 %, respectively, were obtained; 100 % values were found for extra-capsular rupture US diagnosis. US can be used as the first examination in patients with breast implants. US intra-capsular rupture detection requires further evaluation by MRI; in case of extra-capsular rupture US diagnosis, surgical implant removal could be proposed without further investigations.
