Early results with the Moje 1st metatarsophalangeal joint replacement
Patrick Laing & Nilesh Makwana Robert Jones & Agnes Hunt Orthopaedic hospital, Oswestry, England and Wrexham Maelor hospital, North Wales
2
Hallux rigidus: Adolescents & adults affects 1 in 45 adults > 50 yrs. Hartrup & Johnson: Grade 1: mild to moderate osteophytes, joint space preserved Grade 11: moderate osteophytes, narrowing of joint space & subchondral sclerosis Grade 111: marked osteophytes, loss of joint space, plus minus subcondral cysts
3
Treatment Non-operative Cheilectomy Keller resection arthroplasty
Moberg osteotomy Watermann osteotomy Silastic arthroplasty Joint replacement arthrodesis
4
Arthrodesis vs. joint replacement
Range of movement Footwear ? Success rates ? Longevity ? Sesamoid problems ? Metatarsalgia Arthrodesis Strong 1st ray For active lifestyle Usual 80% + success Nonunion Malunion OA ipj
5
MOJE 1st metatarsophalangeal joint replacement
Zirconium ceramic prosthesis Coated with Bioverit 1 – good biocompatibility & osteo-integration
30
44 year old dental technician: previous arthrodesis in excessive dorsiflexion; transfer metatarsalgia and arthritis of ip joint of great toe
31
Ipj fusion united Regained good range of movement of 1st mtpj Very pleased with result
32
Slight cockup deformity
33
Moje + Stainsby 2nd toe Pleased with result – a little stiff initially because of lack of physiotherapy
34
46 year old theatre nurse; previous surgery elsewhere; presented with recurrent hallux valgus & painful 2nd mtpj
36
55 year old lady who had a Moje done elsewhere
38
Very important to get early range of movement exercises
39
Physiotherapy at three weeks
40
Prospective randomised controlled trial comparing arthrodesis to joint replacement
Patients aged 45 years or older RA excluded AOFAS scores Gait analysis looking at 1st mtpj loading To date: 24 patients entered
41
Results Too early for trial results – initial results satisfactory
Overall results 36 Mojes to date 2 revisions of screwfit to pressfit 1 case of early metatarsalgia – now resolving No wound problems No sesamoid problems

Bis 2003 aufgetretene Probleme
Gefahr des Einsinterns eines Implantates Implantatlockerungen Störungen im Bereich des Sesambeinkomplexes Prothesenbrüche durch Bildung monoklinem Zirkoniums

Änderungen des MTP-Gelenk in 2003
Konische Schäfte mit 3° Steigerung Geriffelte Struktur der Anlageflächen zum Knochen Kopf mit 12° Vorneigung (ab ) Plantare Anschliff des Prothesenkopfes Distales Gelenkteil mit exzentrisch 1mm nach dorsal versetztem Schaft Standardisierte AL2O3 – Anteil von 0,5 % in der Zirkoniumkeramik

Ergebnisse der vorgenommenen Änderung
