2 ZIEL DER LEKTION Verwendung von Fallbeispielen um den AHALT Teilnehmer beim korrekten Ausfüllen der Bewohnerfragebogen zu unterstützen Verständnis für die AHALT-2016 Definitionen und HAI Entscheidungsalgorithmen zu schaffen To use case scenarios to aid completion of the HALT-3 resident questionnaire and understanding of HALT-3 definitions and HAI decision algorithms

3 FALL 7 Frau J. Juliet ist 80 Jahre alt und lebt seit 2 Jahren in der Einrichtung, da sie seit dem Beginn von Demenz nicht mehr alleine leben kann. Sie hatte vor 20 Jahren einen Schlaganfall, von dem sie sich vollständig erholt hatte und keine dauerhaften Schäden geblieben sind. Sie ist am Tag der Erhebung um 8 Uhr anwesend. Sie wird medikamentös gegen Demenz sowie prophylaktisch gegen Schlaganfall behandelt. Sie leidet an Bluthochdruck (der medikamentös eingestellt ist) und ist leicht in ihrer Mobilität eingeschränkt, wegen einer Hüftoperation vor 2 Jahren. In ihrer Anamnese ist eine rezidivierende Harnwegsinfektion dokumentiert. J. Juliet, 80-year old female who has been living full time in the LTCF for two years after being unable to live on her own due to the onset of dementia. She had a stroke 20 years ago but made a full recovery with no lasting affects. She is present at 8am on the survey day. She is on medication for the dementia and is also on medication to prevent strokes. She also has high blood pressure (which she is on medication for) and a mobility problem (due to a hip replacement two years ago) for which she has a walking aid. She has a history of recurrent urinary tract infection.

4 FALL 7 Sie hat keinen Harnkatheter, keinen Gefäßkatheter, keinen Dekubitus oder sonstige Wunden Sie ist wegen ihrer Demenz desorientiert, jedoch völlig kontinent. Sie hatte in den letzten 6 Monaten keine Krankenhausaufenthalte. Die von ihrem Geriater/in verschriebene Medikation besteht wie folgt aus: 1xtäglich 2,5 mg Bendroflumethiazid (morgens), 1xtäglich 10 mg Donepezil Hyrochlorid, 1xtäglich mg Trimethoprim per os. Am Tag der Erhebung ( ) wurden keine Veränderungen an ihrem Zustand festgestellt. Sie hat keine Anzeichen für eine erneute Harnwegsinfektion (kein Fieber, keine Dysurie, keine Leukozytose,…) She has no urinary catheter/no vascular catheter/no pressure sores/no wounds. She is disorientated due to her dementia, but fully continent. She has had no hospital admissions in previous 6 months. Her medication regime, as prescribed by her (hospital) geriatrician, is as follows: bendroflumethiazide 2.5 mg once daily (morning), donepezil hyrochloride 10 mg once daily, oral trimethoprim 100 mg once daily. On the day of the survey (2nd Dec 2015) staff have not noticed any changes in her condition. She has no dysuria, no fever, no leukocytosis and no new or increased frequency, urgency, hematuria, or suprapubic pain.

11 FALL 8 Herr S. Shylock ist 72 Jahre alt, lebt seit 6 Monaten in der Einrichtung und wurde am wegen einer Gastroenteritis und schwerer Dehydration in ein Krankenhaus aufgenommen. Ihm wurden ein peripherer Venenzugang und ein Harnwegskatheter gelegt. Er wurde hydratiziert mit Verbessererung der klinischer Zustand. Am zeigt er wieder Beschwerden/Symptome einer Infektion. Diagnose: Pyelonephritis (Escherichia coli >105 cfu/ml) R/ Levofloxacin 500mg per os (1xtäglich, 14 Tage) Am konnte er bei gutem Allgemeinzustand wieder zurück in die Einrichtung transferiert werden. Die Behandlung wird in der Einrichtung fortgesetzt. On the day of the survey (2nd Dec 2015) he has: - No pressure sores or wounds - No vascular or urinary catheters S. Er hatte fallweise kurze Phasen von Desorientiertheit, ist jedoch am Tag der Erhebung orientiert und aufmerksam Zurzeit benötigt er eine Gehhilfe. Antibiotikatherapie (Levofloxacin) am Tag der Erhebung Um zu bestimmen ob die behandelte Infektion mit eines der Falldefinitionen übereinstimmt, sollte überprüft werden ob die Beschwerden und Symptome einer Infektion während der letzten 14 Tage vor der Erhebung vorhanden sind. Der Beginn der Symptome trat 48 Stunden (2 Kalendertage), nachdem der Bewohner in die Einrichtung (wieder-)aufgenommen wurde (außer Infektionen die außerhalb von medizinischen/pflegerischen Einrichtungen erworben wurden und zur Zeit der Aufnahme bereits vorhanden sind oder inkubiert werden). Außer Der Bewohner wurde seit weniger als 48 Stunden aus einer anderen Einrichtung (z.B.: Krankenhaus) in die derzeitige Einrichtung (wieder-)aufgenommen und hat eine Infektion oder wird noch immer wegen der Infektion behandelt. Should a resident questionnaire be completed for S. Shylock? Antimicrobial use (levofloxacin) on the day of the PPS Because of the definition of active HAI the presence of symptoms and signs in the two weeks (14 days) preceding the PPS day should be verified in order to determine whether the treated infection matches one of the case definitions of healthcare-associated infections The onset of symptoms occurred 48 hours (2 calendar days) after the resident was admitted or re-admitted to the LTCF (excluding infections with community-onset already present or incubating at the time of (re-)admission) UNLESS the resident presents with an infection or is still being treated for an infection but has been (re)admitted less than 48 hours after a previous admission to a healthcare facility (e.g. LTCF or acute care hospital).

21 FALL 9 Der 88-jähriger Herr P. Polonius wurde am zum ersten Mal in ein Pflegeheim aufgenommen, nachdem er wegen einer Physiotherapie aufgrund einer Hüftoperation 6 Wochen in einem Rehabilitationszentrum war. Er kann alleine gehen, bewegt sich jedoch die meiste Zeit in einem Rollstuhl fort. Er hatte bisher keine Herz- oder Atemprobleme. Er hat einen Harnwegskatheter und ist stuhlkontinent. Seine Operationswunden sind gut verheilt. Er hat keinen Gefäßkatheter oder Dekubitus und ist sowohl zeitlich als auch örtlich orientiert. P. Polonius, 88-year old male was admitted for the first time to the nursing home on Dec 1st 2015 after he had been in a rehabilitation center for 6 weeks for physical therapy after hip surgery (14 October 2015) He can walk alone but most of the time he moves around in a wheelchair. He has no history of heart or breathing problems. He has a urinary catheter and is continent for faeces. His surgical wound has healed well. He does not have a vascular catheter or pressure sores and is fully orientated.

22 FALL 9 Am Tag der Erhebung ( ) war Herr P. Polonius um 8 Uhr morgens im Pflegeheim. Er hat seit 3 Tagen Husten. Am Tag der Erhebung hatte er eine O2 Sättigung von 92%. Er war ziemlich verwirrt. Seine Körpertemperatur lag am Tag der Erhebung bei 38,3°C. Derzeit wird keine Antibiotikatherapie angewendet. Der Hausarzt untersuchte ihn später an diesem Tag. On the day of the survey (2nd Dec 2015) P. Polonius is in the nursing home at 8am. He has a cough since three days. On the survey day he has an O2 saturation of 92%. He also appears quite confused. His temperature on the day is 38.3oC. He is not on any antibiotics – the GP is due to review him later that day.

31 FALL 10 Die 91-jährige Frau R. Rosalind lebt seit weniger als einem Jahr im Pflegeheim. Sie war am Tag der Erhebung um 8 Uhr morgens anwesend. Sie ist sehr geschwächt, harn- und stuhlinkontinent, bettlägerig und desorientiert. Sie hat einen Sakraldekubitus. Sie hat keinen Harnkatheter, keinen Gefäßkatheter und keine Wunden. Ihr letzter Krankenhausaufenthalt war vor 8 Monaten. R. Rosalind, 91-year old female, has been living in the residential care setting full time for less than a year. She is present at 8am on the survey day. She is very debilitated, doubly incontinent, bedridden and disorientated. She has a sacral pressure sore. She has no urinary catheter/no vascular catheter/ no wounds. Her last hospital admission was eight months ago

32 FALL 10 Am Tag der Erhebung ( ) hatte sie ein Ödem am Unterschenkel. Es fühlt sich heiß an, ist rot und schmerzempfindlich. Am Vortag ( ) verschrieb ihr ihr Hausarzt eine Fusidinsäure Creme (2xtäglich, 7 Tage). On the day of the survey (2nd Dec 2015) she has lower leg oedema of her right leg which is hot to touch, red and tender. The previous day, Dec 1st 2015, R. Rosalind’s GP had prescribed fucidic acid cream for twice daily application for seven days. Fusidinsäure Creme

40 FALL 11 Herr H. Horatio, 68 Jahre, lebt seit 3 Monaten in der Einrichtung und wurde am im Krankenhaus aufgenommen. Wegen seiner Darmkrebserkrankung bekam er eine Kolonresektion. Die Operation und Genesung verlief gut. Herr H. Horatio kehrte am in die derzeitige Einrichtung zurück. Am zeigt die abdominale Inzision eitrige Drainage von subkutanem Gewebe, leichtes Erythem und Verhärtung. Die Inzisionsstelle ist intakt. Eine Probe aus der Wunddrainage wurde ins Labor gesendet (Ergebnis am : Enterobacter spp und E. coli ) Der Hausarzt verordnet dem Patienten für 10 Tage Metronidazol and Levofloxacin per os

41 FALL 11 Am Tag der Erhebung ( ) war er um 8 Uhr morgens anwesend. Er hat keinen Harnkatheter oder Gefäßkatheter. Seine Körpertemperatur ist normal, er ist kontinent und ist sowohl zeitlich als auch örtlich orientiert. Er verwendet einen Rollstuhl um sich fortzubewegen.
